Document Upload Checklist

This page is provided as a reference guide to help you make sure you have all of your documents ready to upload. To prepare your bankruptcy petition, we’ll need a complete set of supporting documents from you. These items allow us to verify your information, assemble your schedules, and ensure your case is filed correctly.

 

Your information is securely handled, encrypted in transit, and never shared outside of what is required to prepare and file your case. You can review our full Privacy Policy anytime.

If you’ve chosen expedited filing, our 24‑hour clock doesn’t begin until we have every required document. This way, you’re never penalized while gathering materials, and once everything is in, we move quickly to get your petition filed.

⚠️ Important Security Notice: We will never ask you to send documents by email. All uploads must be made only through the secure upload link you’ll receive after completing your Case Initiation Documents. Please do not share sensitive files anywhere else.

Documents We Collect

Government-Issued Photo ID

We require a driver’s license or state ID, uploaded front and back, to confirm your identity and ensure your petition matches official records.

Social Security Verification

A Social Security card or verification document, with the full number clearly visible, is needed to confirm your SSN for court filings and credit reporting accuracy.

Emergency Details

A short written description of your emergency — including type, date, and creditor/landlord/employer name — helps us document urgent circumstances like eviction or garnishment and prioritize your filing timeline.

Full Legal Name

Listing all names you’ve used in the past eight years, along with your current address and date of birth (matching your ID exactly), ensures creditors and the court recognize your petition without confusion.

Creditor List

A complete list of all creditors, with names, mailing addresses, and approximate balances owed, allows us to prepare accurate petitions and ensure every debt — credit cards, loans, medical bills, mortgage, car loan — is properly addressed.

Credit Counselling

Federal law requires proof of approved credit counseling within 180 days of filing. Uploading your certificate from an approved provider ensures compliance and keeps your case on track.

Prior Bankruptcy (if applicable)

If you’ve filed before, prior case documents (including case number, chapter, filing date, district, and outcome) are needed to disclose past filings and comply with court rules. If you haven’t filed before, this section can be left blank.

Additional Documents

If you opted for a full filing, we'll collect these documents in a second round of uploads. These financial documents will be used to fill out schedules A-J to ensure all your paperwork is complete and court-ready. 

We understand that not every single one of these uploads will be relevant to every case, but please be sure to upload all documentation that is relevant to you to ensure your petition is complete and accurate.

⚠️ As a BPP, we are prohibited from advising you on asset exemptions. On your upload forms, please indicate which assets you'd like to claim as exempt. You can learn more about common exemptions in your state here.

Last Six Months of Pay Stubs

If you're employed in a salaried position, please upload your most recent six months of paystubs. These verify your current income, and are required for your Means Test, and Schedule I.

Profit & Loss Statements

If you're self-employed or run a small business, upload a month-to-month profit and loss summary for the past six months. This shoes business income, expenses, and earnings for the Means Test, and Schedule I.

Unemployment or Disability Benefits

If you receive unemployment, disability, workers' comp, or similar benefits, upload your most recent award letter and recent payment history. These count as income and must be included on your Means Test calculations. 

Last Two Years of Tax Returns

Upload complete federal returns (including W-2s/1099s). Trustees often require these to confirm income history and ensure all required filings are up-to-date. 

Bank Statements (Last 3 Months)

Upload statements for all checking, savings, and prepaid accounts. These help verify deposits, expenses, and account balances for Schedules A/B and J.

Retirement & Investment Statements

Upload recent statements for 401(k), IRA, brokerage, or pension accounts. These confirm your assets and apply any exemptions you request. They must be listed on Schedule A/B. 

Real Estate Documents

If you own a home or land, upload your deed, mortgage statements, and any appraisal or tax assessment. These help us complete Schedule A/B and apply any exemptions you request.

Vehicle Titles & Loan Statements

Upload your title, registration, and current loan payoff. We use these to record your equity, list debts, and apply any exemptions you request.

Credit Card & Loan Statements

Upload the most recent statements for all credit cards, personal loans, payday loans, and lines of credit. These provide account numbers, balances, and creditor addresses for your creditor matrix.

Collection Letters & Lawsuits

Upload any collection notices, court summons, judgments, or wage garnishment orders. These help us ensure all creditors and legal actions are properly listed. 

Lease or Mortgage Payment Proof

Upload your lease, mortgage statement (if not uploaded previously), or payment history. This verifies your housing expense for Schedule J.

Child Support or Alimony Orders

If you have been court ordered to provide alimony or child support, upload court orders and payment records. These are mandatory disclosures and affect both income and expense calculations.
